What is Primaquine diphosphate?

 

Primaquine Diphosphate is a medication used to treat and prevent malaria and to treat Pneumocystis pneumonia. Specifically it is used for malaria due to Plasmodium vivax and Plasmodium ovale along with other medications and for prevention if other options cannot be used. It is an alternative treatment for Pneumocystis pneumonia together with clindamycin.

 

Function&Application

 

1. This product acts on the infrared phase of plasmodium vivax, and acts on the red phase of chloroquine combined, can cure plasmodium vivax.

2. Mipaline (atapine) and chloroguanidine can inhibit the metabolism of primaquine, so the plasma concentration of primaquine is greatly increased, the maintenance time is prolonged, and the toxicity is increased, but the curative effect is not increased.

3. It is not suitable for use with other drugs that have hemolysis and inhibit the hematopoietic function of bone marrow.

 

This product can kill plasmodium vivax, plasmodium malariae, plasmodium falciparum and plasmodium ovale tissue strains, especially plasmodium vivax. It can also kill gametophytes of various plasmodium parasites, especially plasmodium falciparum, so that it can not develop in the mosquito body, so as to block transmission. The effect of this product on red endozoa is very weak. The antimalarial mechanism of primaquine is not completely clear, which may be related to the interference of DNA synthesis. After the plasmodium and tissue cells were cultured together in primaquine solution for 8 hours, it was observed by electron microscopy that primaquine could change the morphology of plasmodium mitochondria, which was manifested as mitochondrial swelling and cytoplasmic vacuoles. The drug inhibited mitochondrial oxidation and significantly reduced the oxygen uptake of the malaria parasite. Primaquine is metabolized in vivo and transformed into quinoline quinone derivatives with strong oxidation properties, which can transform the reduced glutathione (GSH) in erythrocytes into oxidized glutathione (GSSH). When the latter is reduced, the reduced coenzyme ⅱ (NADPH) is consumed. Coenzyme ⅱ (NADP) has already been consumed during the development of plasmodium in the liver parenchymal cells during the infrared phase. However, primaquine interferes with the reduction of coenzyme ⅱ and reduces the reduction of coenzyme ⅱ, which seriously damages the glucose metabolism and oxidation process of plasmodium.
