Absorption: Studies in normal weight and obese volunteers have shown that absorption of orlistat is minimal and complete orlistat plasma concentration (< 5 ng/mL). The usual therapeutic dose of orlistat is very limited in systemic absorption, non-accumulation, and only occasionally complete orlistat can be detected in plasma at very low concentrations (< 10 ng/mL or 0.02 μm). Distribution: Since orlistat is hardly absorbed, it is difficult to determine its distribution volume, and systemic pharmacokinetics cannot be measured. In vitro, over 99% of orlistat binds to plasma proteins (lipoprotein and albumin are the major binding proteins), and orlistat rarely binds to red blood cells

Metabolism: Animal studies suggest that the metabolism of orlistat is mainly concentrated in the gastrointestinal wall. Studies in obese patients have shown that among the few drug components absorbed throughout the body are two major metabolites, M1(4-cyclolactone ring hydrolysate) and M3(M1 with an n-formylleucine lysate), which account for 42% of total plasma concentrations. M1 and M3 have an open β -lactone ring with very weak lipase inhibition activity (1000 and 2500 times lower than orlistat, respectively). The inhibitory activity and plasma concentration of M1 and M3 were very low (m1-26 μg/mL and M3-108 μg/mL on average) at the treatment dose, so these two metabolites had no pharmacological significance.

Excretion: Studies of normal-weight and obese people have shown that unabsorbed drugs are excreted mainly in the stool. Approximately 97% of the doses administered were excreted from stool, of which 83% were proforma orlistat, with cumulative renal excretion of less than 2% for all orlistat related substances. Complete excretion (stool and urine) of the drug takes 3 to 5 days. Metabolism of orlistat was similar in normal-weight and obese subjects. Orlistat, M1 and M3 can be excreted by bile. The determination of fat content in feces showed that the effect of the drug appeared 24-48 hours after administration, and the fat content in feces returned to the pre-treatment level 48-72 hours after discontinuation of treatment.
