What is  allopurinol  ?

 

Allopurinol is a  xanthine structural isomer of hypoxanthine. Allopurinol inhibits xanthine oxidase, an enzyme that converts oxypurines to uric acid. By blocking the production of uric acid, this agent decreases serum and urine concentrations of uric acid, thereby providing protection against uric acid-mediated end organ damage in conditions associated with excessive production of uric acid, i.e. the massive cell lysis associated with the treatment of some malignancies. Function & Application
 

Allopurinol is a oxidase inhibitor and a widely used medication for gout. Allopurinol is a rare but well known cause of acute liver injury that has features of a hypersensitivity reaction and can be severe and even fatal.

 

Allopurinol is taken as a tablet once a day. It is usually better to take it just after eating and the tablet should be swallowed with water. It is important to drink plenty of water during the day because this will help you get rid of more urate through your kidneys.Your dose of allopurinol may change over time, depending on the amount of urate in your body. Your doctor will monitor your urate levels with blood tests every 2-4 weeks, until they are sure that the dose that you’re taking is high enough to reduce the amount of urate in your body.You may need to remain on a lower dose if you have kidney or liver problems.

Your doctor may recommend that you do not start taking allopurinol until after an attack of gout has passed to avoid triggering further attacks. If this is not possible, it may be started when your inflammation is not too bad.Allopurinol doesn’t treat the immediate pain caused by attacks of gout. But it’s a long-term treatment to get rid of the urate crystals which causes gout attacks. It is likely that you will need to take allopurinol for the rest of your life to manage your urate levels. You should keep taking allopurinol even if: you are experiencing more gout attacks, or it doesn’t seem to help the pain and inflammation at first you stop having gout attacks – stopping your treatment can cause urate crystals to form again, which will lead to more gout attacks.To reduce the effects of gout attacks in the first three to six months of taking allopurinol, your doctor may prescribe a low dose of colchicine or a non-steroidal anti-inflammatory drug (NSAID)​, such as: etoricoxib ketoprofen naproxen .You should be offered another tablet, known as a proton pump inhibitor, to protect your stomach while taking NSAIDs. If you cannot take NSAIDs, colchicine or sterids may be given instead.

1. Adults often take orally. ① Treatment of gout, 0.1g once, 2-3 times a day; Or 0.3g once, once a day; In order to reduce the onset of acute gout, the initial dosage was 0.1g per day, and gradually increased until the serum uric acid concentration approached 6mgJ100ml. The maximum daily dosage should not exceed 0.3g. Because this product metabolite oxypurinol is excreted by the kidney, the dosage of renal insufficiency can be reduced according to the creatinine clearance rate. The daily dosage of creatinine clearance rate is 0.2g when 10 -- 20ml per minute, 0.1g when 3 -- 10ml per minute, 0.1g when less than 3ml per minute, and 0.1g every 24 hours or more. ② Treatment of kidney stones, 0.1 -- 0.2g, 1 -- 4 times a day; Or 0.3g once, once a day.

 

2. Children often take oral dosage. Limited for secondary hyperuricemia of malignant tumors, 50mg once within 6 years old, 3 times a day; From 6 to 10 years old, 100mg once, three times a day, or 300mg once a day. 3. Treatment of major epilepsy: on the basis of using original antiepileptic drugs, take 0.3g of this product in the afternoon every day (0.15g for children under 10 years old every day).

 

This product can induce acute gout and increase the frequency and severity of acute gout, so it should be combined with low dose colchicine in the first 4 to 8 weeks.
